Below is a listing of forms needed to carry out your role as a Troop/Group Leader.
Authorization Agreement for Direct Payment – Use this form to set your account for ACH for product sales.
Trip Approval/Information Form – This form is used prior to taking a trip outside of the council jurisdiction.
Parent Permission Form – Use this form any time your troop leaves their troop meeting place. Remember that sensitive issues, high risk activities and product sales require separate permission slips.
Troop Budget Worksheet - This form is due to the council at the end of each school year. This form provides an update on individual troop finances.
Every girl and adult participating in Girl Scouting must register as a member before participating in Girl Scout activities. Financial Assistance is available.
